Our Transfer Center RNs are available 24/7. They’ll expedite the process by:
- Collecting the information needed to facilitate admission
- Coordinating a call with a Saint Raphael physician, if needed (recorded for accuracy)
- Confirming bed availability and your options if a bed isn’t immediately available
You’ll be asked to provide your patient’s:
- Current location and contact information
- Current medical or surgical need, clinical condition and medical history
