Adolescent Day Hospital

The Adolescent Day Hospital helps the family plan and prepare for discharge by securing ongoing therapy and community services while their child is in the program. The Adolescent Day Hospital hours are 1 to 5 p.m. Monday through Friday. Parents and other family members also attend a weekly family support group on Tuesdays 5 to 6 p.m.

Thousands of procedures are performed each year, including coronary angioplasties, aortic balloon pump support and percutaneous valvuloplasty. The section accepts transfers from other facilities for cardiac diagnostic procedures and emergency cardiac intervention, such as angioplasty. There is a training program in cardiology qualifying cardiac fellows to become certified by the American Board of Cardiovascular Diseases.
Cardiothoracic Intensive Care Unit (Department of Surgery)

The Cardiothoracic Intensive Care Unit (CTICU) is a 16-bed unit where nurses specialize in the immediate post-op management of cardiothoracic surgical patients. The patient population includes those who have undergone surgery for coronary artery bypass graft, valve replacement, ICD implants, myxomas, and aneurysm repairs, as well as those patients who have undergone thoracic and major vascular surgical procedures.

This 32-bed step-down unit, which has the capacity to monitor 20 patients via telemetry, is staffed by nine Board-certified thoracic surgeons, two House Staff officers rotating from the general surgical service and five physician assistants. Nurses specialize in post-op management of cardiothoracic surgical patients. About 1,100 open-heart surgical procedures are performed annually.
Chemical Dependence Program (Evening) (Department of Psychiatry)
Intensive outpatient partial hospital program for individuals who abuse or are dependent on various drugs such as alcohol, cocaine, heroin or prescription drugs. Length of stay based on individual needs and varies from four to eight weeks. Treatment includes individual and family psychotherapy, as well as psychoeducational programs to teach participants how to maintain a substance-free lifestyle. Individuals with concurrent mental illness are treated in this program and followed by program psychiatrists and staff.
Child and Adolescent Mobile Psychiatric Emergency Service (CAMPES)

Offers inhome, community-based crisis intervention services to children and adolescents, ages 3 to 17, and their families. The inpatient unit and Day Hospital have full-time nurses, psychiatrists, pediatricians and clinicians, who provide comprehensive evaluations, including neurological assessments, and short-term treatment.

About 45,000 patients visit the Emergency Department annually, and 25 percent are subsequently admitted. The department is equipped to handle all degrees of illness with the latest therapeutic and technical techniques, as well as to access comprehensive specialty and subspecialty backup. Twenty-four hour ambulatory pH studies and esophageal motilities are also offered.

General Surgery/ENT/Plastic Surgery/Urology (Department of Surgery)
This 32-bed acute unit specializes in the care of general surgery, ENT, plastic surgery, and urology patients.This nursing unit provides pre- and post-operative care of acute general surgical problems. The attending general surgical staff, the surgical house staff and nursing staff care for patients with surgical disease of the digestive, peripheral vascular, and endocrine systems, as well as those patients with injuries resulting from trauma. On the plastic surgery service, nurses participate with the resident and Attending staffs in integrated nursing care. Regarding ENT service, this acute unit takes care of post-operative ear, nose, throat, head and neck surgery patients, as well as those who are sufficiently ill to require hospitalization. Most of these have airway obstruction, wounds that need to be monitored and/or a balance disorder. On Urology service, the Attending staff and a urology physician assistant work with residents and Nursing for care of post-operative kidney, prostate and general urology patients.

The Geriatric Assessment Center evaluates elderly clients for diagnosis and management of medical, psychological, social and functional problems. Physician-referred clients are referred back to physician for follow-up care.

Oncology patients are cared for by a multidisciplinary healthcare team including physicians, nurses, social workers, a hematology/oncology fellow, pastoral care, and other disciplines. The nurses are certified in basic oncology and complete an intense chemotherapy course and practicum. Therapy for patients with leukemia and lymphoma, as well as solid tumors, is provided. Multidisciplinary therapy is coordinated among radiation, medical and surgical oncology. Care of patients with immunosuppression and infection is delivered in conjunction with infectious disease consults. Issues of patient comfort and pain control in the patient with malignancy is coordinated on the unit. Cancer support groups also available.

The HIV inpatient service provides medical consultations for HIV-related and infectious disease problems in HIV-infected patients. Social services, education, pastoral care, and counseling/testing are available. In the Haelen Center, outpatients with HIV and AIDS receive medical care and treatment from a coordinated team of providers that includes physicians, physician associates, nurses, social workers, pastoral counselors, HIV counselors/educators and nutritionists.

Infants with breathing problems or who are at risk for Sudden Infant Death Syndrome are evaluated with polysomnography. The program provides training in the use of the apnea home monitor and infant cardiopulmonary resuscitation. The staff works very closely with the patient’s family and referring physician to ensure a complete evaluation and consistent follow-up after discharge.

This 28-station hemodialysis unit of the regional dialysis center is equipped with state-of-the-art high-flux hemodialysis machines to treat patients with chronic renal failure. Training and follow-up of patients are provided at 136 Sherman Ave. Inpatient peritoneal services are provided on Celentano 3. Inpatient hemodialysis services are provided in the basement of the Main Building.

The Saint Raphael’s-based facility, which also houses the Southern Connecticut Center for the Correction of Dentofacial Deformities, is the leading referral center for the surgical correction of severe jaw and dental abnormalities. The clinic is affiliated with Yale-New Haven, the West Haven Veteran’s Administration and three other hospitals, and is accredited by the American Dental Association. Diagnosis and treatment encompass the diseases, injuries and defects of the oral and maxillofacial region, including surgical treatment of teeth and jaws and dental implant surgery.

Handles referrals for sophisticated orthopedic and neurosurgical procedures including craniotomy; reconstruction; sports medicine; total joint replacements; spinal surgery for traumatic, degenerative and reconstructive disorders; and orthopedic and neuro trauma. Project ElderCare was created to help people 60 years of age and older in underserved areas gain access to primary health care. Services include annual physical exams, treatment of non-emergency illnesses, immunizations, prescription renewals, follow-up, hospital care and referral to specialists. Social services include psychosocial evaluation, individual and family counseling, case management, financial counseling, community resource referrals and advocacy. Transportation to and from Saint Raphael’s for non-emergency visits is provided when applicable. Screenings for high blood pressure, diabetes, vision and hearing problems are offered, as is podiatry. Educational programs for health promotion are also offered.

Patients can maintain family and community relationships while participating in a daily program. Length of stay is usually four weeks. A step-down intensive outpatient program that meets twice a week is also available.

The Sleep Disorders Laboratory provides polysomnographic evaluation of patients suspected of having significant disorders during sleep. Studies are performed on an outpatient basis and require overnight stay in the sleep laboratory. When appropriate, daytime evaluation of excessive sleepiness will be performed.

This 12-bed unit is designed to restore functional mobility and independence of patients who have an illness or injury. Patients impaired with conditions such as stroke or other neurological disorders, arthritis, amputation, trauma, major joint replacement or the immobility problems associated with prolonged hospitalization are commonly admitted. Participation in and benefit from three hours of physical and occupational therapy daily are required for admission. The multidisciplinary team consists of M.D.s, R.N.s, physical and occupational therapists, speech and language pathologists, dietitians, social workers, neuropsychologists and continuing care nurses. Through a concerted effort by patients, their support system and the team, improvement in functional independence, mainly mobility and self-care, is achieved, so that patients can be discharged home.

The TakeHeart inpatient program is designed to meet the educational and activity needs of the newly-diagnosed hospitalized cardiac patient to prepare them for discharge. The multidisciplinary outpatient program for patients with cardiac diagnoses is designed to assist in the recovery process and to encourage lifestyle changes. A nurse, physical therapist, or exercise physiologist, along with the dietitian and social worker, develop an individualized program for each patient. The program consists of an initial physical evaluation and, if necessary, a nutritional and psychosocial evaluation with appropriate follow-up. This is followed by a combination of supervised exercise sessions, private counseling or group education and/or cooking classes. Patients learn about heart disease, stress management, food choices and safe exercise options. Program length varies from four to 12 weeks, depending on the patient’s physiological status and needs. The Adult Fitness Program in Branford helps people to maintain their commitment to good health. A physician referral is necessary.
