Pharmacy Residency Faculty

Trinh T. B?i, PharmD
Trinh earned her Doctor of Pharmacy degree from the University of Connecticut in Storrs, CT.  She completed an ASHP-accredited PGY1 Pharmacy Residency at the Veterans Affairs (VA) Connecticut Healthcare System and an Inter-professional Palliative Care Fellowship at the Bronx VA Medical Center, Bronx, NY.  She is a Clinical Pharmacist and a member of the hospital’s Pain Management Team, working closely with the Bone and Joint Center.

Renee Havriliak, RPh, CGP
Renee earned her Bachelor of Science in Pharmacy at the University of Connecticut and has practice experience in long term care and consulting pharmacy.  She is a certified geriatric pharmacist (CGP) and is currently the manager for the Apothecary & Wellness Center (outpatient pharmacy) at Saint Raphael’s.  Renee is involved in various quality improvement initiatives, including a concierge program to facilitate discharge medications for patients and employee health programs.  She is also a member of ASCP.

JiEun (Christine) Hong, PharmD
Christine earned her Doctor of Pharmacy degree from Rutger’s University and is completing her Master of Business Administration (MBA) from the University of Connecticut.  She completed both her ASHP-accredited PGY1 Pharmacy Residency and her ASHP-accredited PGY2 Pharmacy Residency in Health-System Administration at the Hospital of Saint Raphael with Cardinal Health. She is currently a Pharmacy Operations and Medication Safety Supervisor and is active on several committees, including the Adverse Drug Events and Patient Safety Steering. 

Yu-Hui (Whitney) Hung, PharmD
Whitney earned a Bachelor of Science in Pharmacy at China Medical University, School of Pharmacy in Taichung, Taiwan and her Doctor of Pharmacy degree from Nova Southeastern University, Ft. Lauderdale, FL. She completed a PGY1 Pharmacy Residency at the University of Pittsburgh Medical Center – Presbyterian, Pittsburgh, PA and a PGY2 Infectious Diseases Pharmacy Residency at Tufts Medical Center, Boston, MA.  Whitney has previous experience working as an Academic Pharmacist (Pharmaceutical Industry) in Taipei, providing drug information, clinical seminars, educational programs, and marketing analysis.  She is currently a Clinical Manager, with an emphasis in Infectious Diseases with Cardinal Health, Pharmacy Management. 

Janet M. Kozakiewicz, MS, PharmD
Janet received her Bachelor of Science degree in Pharmacy from the University of Connecticut in Storrs, CT, and Doctor of Pharmacy degree from Massachusetts College of Pharmacy and Health Sciences, Boston, MA.  She also earned her Master of Science degree in Healthcare Administration from Rensselaer Polytechnic Institute, Troy, NY.  Janet is currently the Director of Pharmacy Services for Cardinal Health, Pharmacy Management at the Hospital of Saint Raphael. Janet is a past president of the Connecticut Society of Health-Systems Pharmacists. 

Julie Last, PharmD
Julie earned her Doctor of Pharmacy degree from Massachusetts College of Pharmacy and Health Sciences, Boston, MA.  She completed an ASHP-accredited PGY1 Pharmacy Residency at the Hospital of Saint Raphael with Cardinal Health. Julie is a Clinical Pharmacist and provider in the Anticoagulation and Latent Tuberculosis Clinics.  She is a member of ASHP.  

Erna Lindain, PharmD, MBA
received her Doctor of Pharmacy degree from Rutgers University, in Piscataway, NJ and her Master of Business Administration (MBA) from the University of Connecticut, in Hartford, CT.  She completed her ASHP-accredited PGY1 Pharmacy Residency at Mount Sinai Medical Center, NY and her ASHP-accredited PGY2 Pharmacy Residency in Health-System Administration at the Hospital of Saint Raphael with Cardinal Health. She is currently an Operating Room (OR) Clinical Pharmacist and is active with quality improvement initiatives and committee work, including involvement with the OR nursing committee and Anesthesia Subcommittee. She is a member of CSHP and NYSCHP.

Meghan Mons, PharmD
Meghan earned a Bachelor of Science in Physiology/Neurobiology and her Bachelor of Science in Pharmacy Studies/Doctor of Pharmacy degree from the University of Connecticut in Storrs, CT.  She is a Clinical Pharmacist with experience in critical care and medicine.  She is a member of the hospital’s Heart Failure Task Force and the Falls Prevention Team.

Paul Rochefort, PharmD
Paul earned his Bachelor of Science degree in Pharmacy from the University of Connecticut in Storrs, CT, and received his Doctor of Pharmacy degree from Massachusetts College of Pharmacy and Health Sciences, Boston, MA.  Paul is currently Assistant Director of Operations and Automation, for Cardinal Health, Pharmacy Management at the Hospital of Saint Raphael and serves as a preceptor for automation/patient safety and the longitudinal staffing/Pharmacist in Charge (PIC) experience.   Paul is a member of ASHP and CSHP.

Shusen Sun, PharmD
Shusen earned a Bachelor of Science in Polymer Physics from the University of Science and Technology of China, a Master of Science in Chemistry from the Graduate School of Chinese Academy of Science, a Master of Science in Chemistry from Vanderbilt University, Nashville, TN, and his Doctor of Pharmacy degree from Creighton University, Omaha, NE.  He has previous experience as an Academic Instructor and as a Senior Associate Scientist.  Shusen is currently a Clinical Pharmacist in the Emergency Department, and is active on several committees, including the Radiology and Emergency Medicine Subcommittees.  He works closely with practitioners in the Emergency Department and Child/Adolescent Psychiatry.

Teresa Seo, PharmD, BCPS
Teresa earned her Bachelor of Science degree in Pharmacy from the University of Toledo in Toledo, OH and her Doctor of Pharmacy degree at the Ohio State University in Columbus, OH.  She completed an ASHP-accredited specialty residency in Adult Internal Medicine at the Medical University of South Carolina, Charleston, SC. She obtained board certification as a pharmacotherapy specialist (BCPS).  Teresa is currently Assistant Director of Clinical Services, and Senior Clinical Manager with Cardinal Health, Pharmacy Management.  Teresa is a member of CSHP, ASHP, ACCP, and ASCP. 

John Tarutis
John is the Executive Director for the Sister Anne Virginie Grimes Health Center, and he is a preceptor for the Long Term Care pharmacy experience.   He has a Bachelor degree in Business Administration for Quinnipiac University. He is past President of the American College of Health Care Administrators. Mr. Tarutis also is administrative director for the Inpatient Rehabilitation Unit located within the hospital. He serves as part of the senior management team of Saint Raphael Healthcare System (SRHS) and is a member of the SRHS Quality Committee and Ethics Committee.

Beth Wilson, PharmD, BCOP
Beth earned a Bachelor of Arts degree in Biochemistry from Albertus Magnus College, New Haven, CT.  She also earned both her Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees from the Massachusetts College of Pharmacy and Health Sciences.  She obtained board certification in oncology pharmacy (BCOP).  Beth serves as a Clinical Manager specializing in Oncology at the Hospital of Saint Raphael and is active on several committees, including the Drug Research, Nephrology, and Oncology Subcommittees.  She is a member of ASHP and ACCP. 

Agnes Zajac, PharmD, BCPS
Agnes earned a Bachelor of Science in Nursing from University of Connecticut, Storrs, CT.  She also earned both her Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees from University of Connecticut, Storrs, CT.  She completed an ASHP-accredited PGY1 residency at Virginia Commonwealth University Health System/Medical Center in Richmond, VA and a PGY2 residency in Internal Medicine at Yale New-Haven Hospital in New Haven, CT.  She obtained board certification as a pharmacotherapy specialist (BCPS).  She is currently a Clinical Manager, with an emphasis in Critical Care with Cardinal Health, Pharmacy Management.
