Diabetes Healthcare Program
The Diabetes Healthcare Program at the Hospital of Saint Raphael is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.
The program consists of four (4) two and a half hour (2?) hour sessions covering a variety of topics related to Diabetes Self-Management. The classes are offered once a week, weekday mornings (9:00 – 11:30 a.m.) or afternoons (3:00 – 5:30 p.m.) for four consecutive weeks as outlined below:

The classes are taught by Leigh Bak, MS, RN, CDE, Sandy Fillion, MS, RN, Cindy Fitteron, RD, CD-N and Kelly Ford, LCSW. In addition to the four standard classes offered through the program, individual sessions to learn a specific skill (such as: insulin administration, blood glucose monitoring) can also be arranged.
To enroll in the standard program or schedule an individual session, please call 203-789-3391 with your name, address, phone number and the name of your physician. A physician referral is required. Check with your health insurance company regarding coverage of the fees for diabetes education.
Individual meal planning sessions can be arranged through the Hospital of Saint Raphael Nutrition Clinic by calling 203-789-3266 to schedule an appointment with a dietitian.
