If you or your loved has diabetes, you may attend the four-week Diabetes Healthcare Program.  The Diabetes Healthcare Program at the Hospital of Saint Raphael is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.  A physician referral is required.  

The program consists of four (4) two and a half hour (2?) hour sessions covering a variety of topics related to Diabetes Self-Management. While there is a fee, the cost is coverd by most medical insurance plans.  The classes are offered Wednesday mornings (9:00 – 11:30 a.m.) or Tuesdays afternoons (3:00 – 5:30 p.m.) for four consecutive weeks as outlined below:

Class 1:

  What is Diabetes?

– Diagnosis of Type 1 & Type 2 Diabetes

– Treatment Overview

– Self Monitoring of Blood Glucose

Class 2:

  Nutrition & Meal Planning;

– Nutrient Groups

– Healthy Eating Habits

– Label Reading

 

Class 3:

  Exercise

– Medications & Insulin

– Preventing and Treating Hypo-
glycemia (Low Blood Sugar)

Class 4:

  Hyperglycemia (High Blood Sugar)

– Sick Day Management

– The Link Between Control & complications

– Healthy Habits to Prevent Diabetes Complications

The classes are taught by Leigh Bak, MS, RN, CDE, Donna Greenberg, APRN, Cindy Fitteron, RD, CD-N and Kelly Ford, LCSW.  In addition to the four standard classes offered through the program, i ndividual sessions to learn a specific skill (such as: insulin administration, blood glucose monitoring) can also be arranged.

To enroll in the standard program or schedule an individual session, please call 203-867-5695  with your name, address, phone number and the name of your physician. A physician referral is required. Check with your health insurance company regarding coverage of the fees for diabetes education.

Individual meal planning sessions can be arranged through the Hospital of Saint Raphael Nutrition Clinic by calling 203-789-3266 to schedule an appointment with a dietitian.
