VNA Services, Inc., (www.vnaservices.org) is a wholly-owned Saint Raphael Healthcare System subsidiary that provides homecare services to residents of 34 Greater New Haven, Middlesex County and shoreline communities. The agency provides nurses, home health aides, homemakers, occupational, speech and physical therapists, medical social workers, wound care specialists, telemonitoring nurses/specialists, registered dietitians and a certified diabetes educator. These healthcare professionals work as an interdisciplinary team to meet the healthcare needs of patients in their homes.

Patients are referred to VNA Services by a variety of hospitals, physicians and other sources. Patients who already use the Saint Raphael Healthcare System may select VNA Services for their homecare needs to ensure continuity of care or they can choose another home care provider.

VNA Services offers a variety of healthcare and support services. Using a multidisciplinary team model, staff provides in-home management and coordination of healthcare in collaboration with a patient’s attending physician. Patients may receive a single type of skilled care or a combination of services, depending on the complexity of their needs.
