Going HOME
Located on Celentano 3, Saint Raphael’s Going HOME program assists people 65 and older hospitalized for acute medical conditions that are complicated by other physical, psychological or emotional issues. While your primary care physician treats your acute illness, our team of geriatric care specialists works with you and your physician to address your special needs.
Program patients typically have acute illnesses such as:
- Pneumonia
- Chronic obstructive pulmonary disease
- Diabetes-related illnesses.
In addition, they may have:
- difficulty performing one or more of the activities of daily living
- trouble walking or a history of falls
- depression.
Our goal is to help with these challenges and maintain your physical and mental capabilities while you are hospitalized, so you can return directly home. For most patients, this means a short-term hospital stay.
Our clinical team includes:
- Geriatricians
- Nurses
- social workers
- physical, occupational and speech therapists
- care coordinators
- dietitians
- pharmacists
- pastoral care staff.
This team meets daily to discuss your needs and makes specific recommendations to your primary care physician to address them.
