Visiting nurses/home care services
VNA Services’ visiting nurse program offers a variety of health care and supportive services,
consistent with a plan authorized by the patient’s physician. A primary nurse works with the
patient’s attending physician to monitor patient status and facilitate timely, appropriate
outcomes. Patients may receive a single type of skilled care or a combination of services,
depending on the complexity of their needs. Visiting nurse services include:
- Skilled nursing – For the administration of therapy, patient assessment and prevention/treatment education in the home.
- Home health aides – To assist patients with activities of daily living and routine personal care and hygiene, exercise and nutrition.
Nursing services are available 24 hours a day, 365 days a year.
Medical social work
VNA Services’ social worker helps patients and their families deal with the emotional stress
associated with disease and treatment. These services are appropriate for patients who have
social or emotional problems that adversely affect their medical condition, treatment or
recovery. Services include:
- Evaluation of social, economic, environmental and emotional factors that interfere with treatment
- Counseling for decision-making and long-range planning
- Education/counseling pertaining to the identified medical condition
- Identification of necessary community resources
- Short-term therapy
- Crisis intervention
- Consultation, liaison and interdisciplinary collaboration
- Advocacy for services for patients at risk due to mental or physical limitations
Rehabilitation services
VNA Services offers comprehensive rehabilitation services to help patients reach optimal
functional status and level of physical ability. These services include rehabilitative
nursing, medical social work, and physical, speech and occupational therapy. Rehabilitation
nurses are responsible for the organization of the overall care plan for each patient. Patients
may also receive additional services if needed, including medical nutrition therapy provided by
a registered dietitian, behavioral health and home health aide services.
Patients suitable for the rehabilitative services program include those recovering from:
- Cardiovascular disease
- Traumatic brain injury
- Joint replacement
- Neuromuscular conditions
- Arthritis
Wound care program
The program’s risk assessment system promotes regular, ongoing evaluation of a patient’s risk
for developing chronic wounds and facilitates timely intervention. Accurate and quantifiable
assessments of wounds guide treatment decisions and help evaluate effectiveness of care.
Wound specific patient teaching plans are developed with a sequential teaching schedule that
has measurable, objective goals and outcomes.
The Strong House
The Strong House serves adults in a social and healthcare environment, either for the entire
day or a part of it. The tranquil and welcoming atmosphere of The Strong House is enhanced by
its location on more than two wooded acres in a beautiful country setting at 546 Durham Road in
Madison. Services include:
- Meals
- Nursing
- Counseling and support groups for patients and caregivers
- Recreational activities
- Transportation to and from Clinton, Guilford, Killingworth and Madison
- Assisted showers, hairdressing and podiatry
