Using the latest national research and monitoring our own patient
safety efforts, Saint Raphael's develops policies and procedures to
meet our patients' specific safety needs and ensure continual
improvement of our programs and services.
Patient Safety reporting
Patients and visitors are asked to report any safety concerns
to their caregivers first, or to Patient Relations, (203) 789-3508.
Saint Raphael staff members and physicians report any real or potential
safety issues through the use of safety variance forms, by contacting
Saint Raphael's Risk Management department or by using the a Patient Safety
Hotline. Saint Raphael's will soon institute a Web-based reporting system to
ensure prompt reporting of safety variances. Prompt reporting allows us to
make improvements, which prevent some problems and correct others before they
affect patients.
Internal research and analysis
Teams of Saint Raphael staff members assess
the safety and efficiency of procedures and processes.
