Saint Raphael's is the area's provider of choice for seniors.
We offer a range of services to promote better health and to
help people maintain independence as they age.

- CareCard:
CareCard: a FREE health and wellness membership program for people
55 and older that offers everything from activities like Aquacise to
screenings for blood pressure and cholesterol. With more than 23,000
members, it's the largest program of its kind in Connecticut. A
Spanish
version is also available.
- Senior Assessment Centers
to help patients and families meet the
challenges of independent living.
- Senior
outreach to provide patients and physicians with information,
consultation and referrals to important community healthcare services.
- Project ElderCare,
which brings health care to elderly residents who
might otherwise have difficulty accessing services.
- Healthy Aging Line
Healthy Aging Line for easy access to Saint Raphael's wide array of senior services.
The following programs are provided by related entities of,
in affiliation or partnership with, or endorsed by the Hospital of
Saint Raphael:
- Lifeline,
a 24-hour personal security and emergency response program endorsed
by the Hospital of Saint Raphael.
- Sister Anne Virginie Grimes Health Center
(formerly known as Saint Regis)
the only Catholic skilled nursing home in New Haven.
- Homecare
partner
VNA Services, Inc., to provide quality
care at home following, or instead of, a hospital stay.
- Parkinson's Center, a place for support for those with Parkinson's
disease.
