hundreds of seniors in easy-to access community settings, including elderly housing complexes.

Project Eldercare offers:

- Complete geriatric primary care
- Opportunities to develop long-term relationships with permanent staff, including geriatric physician, physician assistant and social worker
- Social services, including counseling for personal and financial problems
- Labs drawn at clinic
- Foot care by podiatrists
- Less wait for follow-up appointments
- Coordination of care with your medical
specialist.
- Patients can be seen on other days at other clinic sites for urgent problems
- Health screenings including blood pressure, blood sugar, vision and hearing
Eligibility criteria
To participate in Project ElderCare, you should be age 60 or older and a New Haven or West Haven resident.
