Senior Assessment Centers
As people age, they may develop health problems that can pose challenges in day-to-day life. Saint Raphael's Senior Assessment Center can help. We offer a caring and compassionate team of healthcare professionals to:

Assess each patient's health and functioning level

Develop an individualized care plan that addresses the needs of both the patient and family

Support patients and their families in finding the most appropriate levels of care, while maintaining as much independence as possible.
Patient teams may include a board-certified geriatric physician, a geriatric nurse specialist and a social worker. Physical, occupational and speech therapists are on-call for consultations, along with physiatrists, dietitians and podiatrists.
