Anticoagulation Clinic & Latent Tuberculosis Clinic

Eligible patients actively seen in the Adult Primary Care Clinic may be referred by their physicians to the Anticoagulation Clinic to receive patient education, international normalized ratio (INR) point-of-care testing, and warfarin therapy management in collaboration with the patient's primary care provider.

Eligible patients actively seen in the Family Health Center may be referred by their physicians to the Latent Tuberculosis Clinic to receive patient education and follow-up regarding prescribed tuberculosis medications.

Concierge Pharmacy Service

The Hospital of Saint Raphael's "Concierge Pharmacy Service" now coordinates outpatient medications at the bedside. Saint Raphael's Concierge Pharmacy Service will fill your prescription on discharge, giving you one less stop to make on your road to recovery.
