Family Health Center

The Hospital of Saint Raphael Family Health Center and Ambulatory (meaning outpatient treatment on the same day) Specialty Clinics offer a range of primary health care services and medical specialty services.  Located just off the "S" Elevators on the 2nd floor of the Main hospital building, the Family Health Center provides comprehensive primary health care to patients throughout their lifecycle. Services include:

  • Anticipatory guidance, developmental screening, and health education
  • Chronic disease management
  • Collaboration with community-based providers, including rehabilitation services, specialty care providers, and home health agencies
  • Coordination of the continuum of care, including referrals to specialists, admissions for in-patient services, and the establishment of post-discharge treatment plans
  • Diagnosis and treatment of acute and episodic illnesses
  • Prenatal and women's health services
  • Primary care (prevention) services
  • Well-baby and well-child care
  • Preventative dental care for children ages one to 6

The Family Health Center's clinical team includes: internal medicine physicians, pediatricians, obstetricians/gynecologists, nurse practitioners, certified nurse-midwives, physician associates, pharmacists, and physicians who provide a wide range of medical specialties including: pulmonology, endocrine/metabolic, renal (kidney), and rheumatology (joint and muscular system disorders).

Insurance and finanacial assistance

Our primary care clinicians participate in Connecticut's four HUSKY A Medicaid managed care plans, as well as fee-for-service Medicaid, and the SAGA program. Financial counselors provide assistance to uninsured patients about various publicly- funded insurance programs and other sources of coverage. Our patients have access to a number of supportive services, such as social work, diabetic education, a medication assistance programs, nutritionist, WIC, and family literacy promotion.

Within the Ambulatory (Outpatient) Surgical Specialty Clinics on the second floor of the Orchard Medical Building (330 Orchard Street), patients have access to a variety of specialty services. The clinical team includes general surgeons, orthopedic surgeons, urologists, vascular surgeons, podiatrists, otolaryngologists and plastic surgeons.
