We frequently get requests for copies of birth certificates. The Hospital of Saint Raphael does not keep birth certificates or copies on file.

If you were born at Saint Raphael's and need a copy of your birth certificate, please contact the New Haven Office of Vital Statistics, 165 Church St., New Haven, CT 06511, (203) 946-8084.  Their office hours are:  Monday thru Friday, 9:00 a.m. to 4:30 p.m.
