- Responsible for improving patient flow throughout the entire delivery system, facilitating patient bed placement, managing length of stay and facilitating patient discharge and transfers to support the desired clinical and financial outcomes of the the Hospital. Facilitates processes on a daily basis to expedite efficient patient movement throughout the continuum of care. Functions as the liaison and works in collaboration with the multi-disciplinary team and a variety of internal and external sources to meet patient flow goals and objectives and to resolve issues and problems. Provides overall direction, guidance and coordination to health care providers in the achievement of patient movement. Registered Nurse licenses by the State of Connecticut. Bachelor's degree in health related field preferred. At least three (3) years relevant hospital experience to include a clinical background in diversified medical/surgical roles. Experience in the areas of discharge planning, utilization review and quality assurance a plus. Thorough knowledge of medical and ancillary services, appropriate levels of care, treatment and modalities and the healthcare delivery system.
Hours: 7:30a.m.-4:00p.m. (salaried position) Monday through Friday to start, will eventually transition to 12 hour/days (7:30a.m.-8:00p.m.) with alternate weekends.
